Checklist and Points to Note

Before you start filling in the application form, make sure you have all the relevant documents and dates to hand – you will need all of this information to complete this form.

Online Application Checklist

  • Date of Accident.
  • Details of the respondent – the person(s), entity or entities that you are making the claim against.
  • Credit card or debit card details. A fee of €45 is payable to process your claim.
  • A completed medical report (if available).

Important

If you do not have a medical report right now you can submit your claim without it and send it later.

Remember

The respondent (the individual(s), entity or entities you are making a claim against) will be notified that you are making a claim against them once you have submitted your completed application. InjuriesBoard.ie will also contact you directly once the form has been received to follow-up on details of your case and progress the application with you.
